    Maintenance and Relapse After TMS

What happens after TMS treatment ends? Understanding maintenance sessions, relapse, and long-term follow-up.

    What happens after treatment ends

After completing a course of TMS or SAINT, your care team schedules follow-up assessments — typically at one week, one month, and beyond. These check-ins help track how you're doing and whether any additional steps are needed.

    Maintenance TMS

Some people benefit from maintenance or "booster" TMS sessions after the initial course. Maintenance TMS involves periodic sessions — weekly, monthly, or as needed — to sustain the benefits of the initial treatment.

Whether maintenance is appropriate for you is a clinical decision. Note that some insurance plans, including FEP, classify maintenance or continuation TMS as investigational, which may affect coverage.

    If symptoms return

Depression can return after any treatment, including TMS. If symptoms come back, it doesn't mean the treatment failed — it means the depression returned, which is a known feature of the condition.

If symptoms return after TMS, options include:

    • A repeat course of TMS (insurers may require a waiting period between courses)
    • Maintenance TMS sessions
    • Adjusting your medication plan with your prescriber
    • Exploring a different treatment — SPRAVATO, ketamine, or another option
    • Intensifying psychotherapy

    For SAINT specifically

After SAINT, your care team checks in to see how you're doing and discusses next steps. Maintenance or follow-up treatments may be recommended. You should continue seeing your psychiatrist, and talk to them before stopping any medications.
